Server Error in '/' Application caused by lack of IIS Application

When attempting to load Ultimate Survey Enterprise survey software, the application crashes with the following message:

Server Error in '/' Application.

Configuration Error
Description: An error occurred during the processing of a configuration file required to service this request. Please review the specific error details below and modify your configuration file appropriately.

Parser Error Message: It is an error to use a section registered as allowDefinition='MachineToApplication' beyond application level. This error can be caused by a virtual directory not being configured as an application in IIS.

This is the result of an improper IIS Application configuration.

To remedy the situation:
- Open the IIS Manager
- Browse to the web site and folder that your Ultimate Survey installation is in
- Open the properties for this folder
- On the "Directory" tab, there is a section that says "Application Settings"
- Press the "Create" button and apply the changes
- Try to open the application again
